Opportunities for Giving

Remember a loved one by making a gift in his/her name. Simply tell us the name of your loved one and whom you would like us to acknowledge. Your gift can be a donation or in the form of a memorial.

Gifts in Kind

Donations of patient, office, or bereavement supplies, and fundraising event prizes can be given throughout the year. Contact our office at (815) 756-3000 or look at the Wish List in our newsletter to find out our specific needs.

Why your support helps:
Our expenses are covered by a per diem from Medicare, Medicaid, private insurance or private pay. In many cases the expenses are greater than the reimbursement we receive. We also offer programs that are totally dependent on donations. Our Bereavement Program is committed to providing services which support and educate individuals who have experienced the death of a loved one. This program is offered to anyone in the community.Our Transitions Program offers support to individuals and their families who have a life limiting illness but are not yet ready for hospice.
