What is Transitions?

Transitions offers assistance, encouragement and support to individuals and their families who have a life limiting illness with an expected prognosis of a year or less.

Transitions services are offered at no charge to the residents of DeKalb County and surrounding communities.

Individuals are eligible for the supportive services of Transitions with the consent of their physician and regardless of their current medical treatment. All information is treated confidentially.

Transitions Volunteer

Transitions uses specially trained volunteers who provide extra support to the Transitions client and their family.

How to Get Help
Referrals can be made by the individual, family, friends, doctors, nurses, and other professionals. Call (815) 756-3000 to start the process. The Transitions Coordinator is a client advocate who will work with the client and family. Together they will identify needs and the coordinator will offer assistance to fulfill those needs with referrals and links to community resources.
